Eligibility

NHS eye tests are provided to patients who are normally entitled to an NHS eye test but are unable to leave their home due to a condition that stops
them from leaving their home. This can be a mental or physical condition.
You may be eligible for a free NHS home eye test, if you or someone you know is:

Aged 60 or over
Registered blind or partially sighted
Diagnosed with diabetes or glaucoma
receiving benefits (income support, Income-based Jobseekers Allowance, Income-related Employment and Support Allowance or Pension Credit Guarantee Credit)
Aged 40 or over and your mother, father, brother, sister, son or daughter has been diagnosed with glaucoma
Entitled to, or named on, a valid NHS tax credit exemption certificate
On a low income and named on a valid HC2 or HC3 certificate
